A: Golden Apple Services is an independent case management company founded by a passionate and experienced A&D waiver case manager with a focus on client-centered, communicative, and collaborative care. We are currently accepting new clients and are excited to become acquainted with you, build a relationship, and help you receive the services you need to remain safely in your home. If this sounds good to you, then Golden Apple is a good fit.

Q: Are you a prospective waiver applicant who is interested in applying for waiver?

A: The Indiana Division of Aging has not approved independent case management companies, such as Golden Apple Services, to complete initial assessments for waiver. Despite this, we would be happy to speak with you and point you in the direction of your local Area Agency on Aging to begin the application process. After you are approved for waiver, you can request to become a client of Golden Apple.

Q: Are you a waiver client whose primary need is low-income housing?

A: Golden Apple does not provide housing. We can, however, help you get into a waiver-approved assisted living community if you are eligible. If you are not interested in assisted living, we can direct you to other housing resources, such as your local housing authority or township trustee. We are happy to provide you with resources to help you in your search for housing; however, our primary area of case management is A&D and TBI waiver, not housing.
